  • 24-hour on call support provided through Phoenix resources
  • support provided based on an individual assessment of needs/wellness plan – may include: skills of daily living, medication management, financial trusteeship, leisure/social activities, psychiatric support, crisis intervention and other support services
  • “menu of services” ranges from outreach (minimal) to moderate level of support; focus is on the development of support networks in the community and less reliance on formal system as per the individual Needs Assessment & Wellness Plan
  • staff support is available Monday – Friday from 8:30-5:00pm
